How to use it

  1. Print the page, or use your browser’s “Save as PDF”.
  2. Take two readings in the morning and two in the evening, around the same times.
  3. Write down the date, time, and your systolic / diastolic / pulse.
  4. Bring the filled-in sheet to your next appointment.
